Benefit Eligibility Assessment

Determining benefit eligibility for long term care insurance is a complex process, beginning with a comprehensive and objective evaluation of an individual’s status. Univita has the most skilled and experienced experts in evaluating functional and cognitive abilities especially related to long term care needs.

Beyond the initial assessment, Univita can develop a thoughtful plan of care that specifically outlines the support an individual requires. Further, Univita monitors ongoing needs and modifies the plan of care to reflect the most current situation.


  • CareLink Network

    Certain assessments often require an in-person interaction. Our network is comprised of registered nurses who specialize in functional health and perform a wide range of in-person assessments with individuals wherever they may be – in a home, care facility or institution.

    Our nurses have backgrounds in care plan development and social work and are located in all 50 states, Canada, Puerto Rico, and the Virgin Islands. All nurses in the network undergo a rigorous selection process, background reviews and continual quality assurance.

  • Cognitive Screening

    As people age, their cognitive abilities change, which dramatically impacts their daily lives. The MCAS (Minnesota Cognitive Acuity Screen) is the most proven and accurate screen for cognitive impairment. Unique to Univita, the MCAS can be performed in about 15 minutes either in person or over the phone, and has been validated as the most comprehensive screen, evaluating multiple cognitive domains.
